Job Description:
A company is looking for a part-time After Hours Care Manager to
provide telephone-based care management services.
Key Responsibilities:
Handle inbound communications from patients regarding symptoms or
care concerns
Consult and coordinate with team members to assess and implement
patient care plans
Record member data in the company's web-based medical record
system
Required Qualifications:
Registered Nurse (RN) with current compact licensure in assigned
state(s)
Bachelor's degree, at minimum
Minimum 3-5 years of recent clinical experience
Experience in a call center, triage position, or similar
environment
Willingness to maintain a flexible schedule, including nights and
weekends
