Responsibilities:
Position Overview

our dedicated team as a Care Coordinator at Newport Healthcare

As a Care Coordinator, you will play a vital role in ensuring the
safety and well-being of our clients while building meaningful
relationships with them. You will be responsible for supervising
clients, facilitating daily activities, and documenting their
progress. Your ability to communicate effectively and provide
compassionate support will be essential in creating a positive
environment for our residents.
Sign-On Bonus

Key Responsibilities
Maintain awareness of client activities and whereabouts at all
times, ensuring their safety through vigilant supervision.
Utilize verbal de-escalation techniques to redirect behaviors and
respond to crises, employing Crisis Prevention Institute techniques
when necessary after completing training.
Facilitate daily schedules and activities, group participation,
meal service, and community outings, while actively participating
and providing support.
Document daily shift notes and communicate client concerns with the
treatment team to ensure comprehensive care.
Provide transportation for residents to community events and
meetings in a safe and legal manner.
Attend weekly clinical supervision and remain current on all
required trainings and certifications.
Qualifications:
Must be 21 years of age or older.
High school diploma or GED required
Bachelor's degree in psychology, counseling, or sociology
preferred.
Valid driver's license and eligibility for insurance coverage for
driving company vehicles.
CPR certification required within the first 30 days of
employment.

**Pay range for this position is $18 to $22 depending on education
and experience.
We are committed to providing competitive and equitable
compensation. An individual's pay rate within the posted range is
determined by several factors, including skills, experience,
education, and market conditions. Additionally, we consider
internal equity to ensure fairness within our organization.
